目的 探讨疾病感知和社会支持在老年COPD患者自我调节疲劳与老化期望间的链式中介效应。方法 采用便利抽样法，于2023年1月至12月选取安徽省某医院呼吸科收治的149例老年COPD患者作为调查对象。采用一般资料调查表、自我调节疲劳量表、简易疾病感知问卷、社会支持评定量表、老化期望量表对其进行调查。结果 老年COPD患者自我调节疲劳得分为(53.57±15.83)分、疾病感知得分为(45.92±14.83)分、社会支持得分为(31.00±9.93)分、老化期望得分为50.00(29.00,61.00)分；老化期望与自我调节疲劳呈负相关(r=-0.876,P<0.01)、与疾病感知呈负相关(r=-0.877,P<0.01)、与社会支持呈正相关(r=0.895,P<0.01)。中介效应分析显示，自我调节疲劳对老化期望的总间接效应为-0.682,疾病感知和社会支持的中介效应分别占总效应的43.10%、24.50%,二者的链式中介效应占总效应的8.35%。结论 老年COPD患者疾病感知和自我调节疲劳处于较高水平，老化期望和社会支持处于较低水平，疾病感知和社会支持在自我调节疲劳与老化期望间的多重中介效应成立。建议医护人员积极改善老年COPD患者的疾病感知，提升社会支持水平，降低其自我调节疲劳，进而提高老化期望水平。
Abstract：Objective To explore the chain-mediating effect of illness perception and social support between self-regulatory fatigue and aging expectations in elderly patients with COPD.Methods Using the convenient sampling method, a total of 149 elderly patients with COPD were investigated using the General Information Questionnaire, Self-Regulatory Fatigue Scale, Brief Illness Perception Questionnaire, Social Support Rating Scale, and Expectations Regarding Aging Scale.Results The mean scores of self-regulatory fatigue, illness perception, social support, and aging expectations in elderly patients with COPD were(53.57±15.83),(45.92±14.83),(31.00±9.93),and 50.00(29.00,61.00),respectively.Aging expectations were negatively correlated with self-regulatory fatigue(r=-0.876,P<0.01) and illness perception(r=-0.877,P<0.01),but positively correlated with social support(r=0.895,P<0.01).Mediation analysis showed that the total indirect effect of self-regulatory fatigue on aging expectations was-0.682.Illness perception and social support individually accounted for 43.10% and 24.50% of the total effect, respectively, while their combined chain-mediated effect contributed 8.35% to the total effect.Conclusion Elderly patients with COPD exhibit high level of illness perception and self-regulatory fatigue, but a low level of aging expectation and social support.The chain-mediated effects of illness perception and social support between self-regulatory fatigue and aging expectations are statistically valid.Medical staff should actively improve patients' illness perception, enhance social support, reduce self-regulatory fatigue, and thereby elevate aging expectations.
