目的 调查湖南省“安宁疗护病房管理规范”地方标准的实施现状并分析影响因素，为规范安宁疗护病房建设、完善规章制度提供参考依据。方法 采用自行编制的调查问卷，对湖南省93所医疗机构的安宁疗护病房管理规范制度建设和实施情况进行调查。结果 93所机构中，86.02%的机构制订了地方标准相关制度、77.42%设立了主管部门、54.84%配备专职人员负责宣传与落实“安宁疗护病房管理规范”地方标准，93名管理者对标准内容全部了解为79.57%。地方标准实施得分为3.68(3.03,3.93)分，处于中等水平。其中，安全管理维度得分最高，人员管理维度得分最低。多元线性回归分析显示，医疗机构是否制订“地方标准”宣传与实施制度(P<0.05)、医疗机构是否有负责“地方标准”宣传与实施人员(P<0.01)是“安宁疗护病房管理规范”地方标准实施的主要影响因素。结论 湖南省安宁疗护病房管理已初步建立并完善了各项制度，但医疗机构是否制订地方标准宣传与实施制度、医疗机构是否有地方标准宣传与实施人员仍是薄弱环节。建议从制度建设、专职岗位设立、分层培训和量化评估等方面系统推进，进一步提升标准实施效能。
Abstract：Objective To investigate the implementation status and influencing factors of the local standard "Management Specifications for Hospice Care Wards" in Hu'nan Province,and to provide references for standardizing hospice care wards construction and refining relevant regulations.Methods A self-developed questionnaire was used to investigate the system construction and implementation status of hospice care ward management standards across 93 medical institutions in Hu'nan Province.Results Among 93 institutions,86.02% had established systems related to the local standard,77.42% had set up dedicated management departments,54.84% had designated dedicated personnel for promoting and implementing the standard,79.57% of the 93 managers demonstrated a comprehensive understanding of the standard content.The score of the local standard was 3.68(3.03,3.93),indicating a moderate level of implementation.Among the evaluation dimensions,the score in the dimension of safety management was the highest,whereas the dimension of personnel management was the lowest.Multiple linear regression analysis showed that whether medical institutions have established systems for promoting and implementing the local standard(P<0.05) and whether medical institutions have dedicated personnel responsible for promoting and implementing the local standard(P<0.01) were the main infiuencing factors for standard implementation.Conclusion Hu'nan Province has made initial progress in establishing and refining systems for hospice care ward management.However,whether medical institutions have established systems and whether medical institutions have dedicated personnel responsible for promoting and implementing the local standard remain weak links.It is suggested to systematically advance the work including system construction,establishment of full-time positions,hierarchical training,and quantitative evaluation to enhance standard implementation effectiveness.
